Health-system pharmacists are employed by hospitals, health maintenance organizations, long-term care facilities, home care and other free-standing medical centers and clinics nationwide. Many pharmacists who practice in this setting have completed a residency program and work to fully utilize their clinical skills. The role of a pharmacist is changing to include more clinical knowledge and consultative skills as to keep up with the changing needs of the health-system industry.

Responsibilities for a hospital pharmacist include: administrative, teaching, preparing and revising of formularies, mixing sterile solutions to be administered intravenously, monitoring drug regimen, and participating in the pharmacy and therapeutics committee. Pharmacists who practice in a home health setting are responsible for preparing medications and monitoring drug therapy for use in the home.
